Below, is the printed copy of the Diver Certifications page. It includes the divers "C" card and all other certifications the diver has entered. (All forms are printed on a half sheet of paper (5.5 x 8.5/105x148) for use in standard hard copy dive log books, and can be printed in color, or black and white)
